This is a responsible, non-supervisory position requiring familiarity with various veterans/government programs and insurance coverage related to services for veterans. Duties include, but not limited to: Implements and coordinates the case management in the region to ensure prescribed activities are carried out in accordance with specified objectives of the Veteran Directed Care Program; Provides initial assessment, technical assistance and develop spending plans for consumer directed services; Conducts and documents ongoing follow-up contacts and home visits.
Responsible to handle, mediate and resolve disputes in regards to services, clients and provider issues; Conducts assessments; Provides counseling; Tracks and verifies expenditures; Coordinates with the Fiscal Management Agency; Provides resource information or referrals to veterans and/or their family members; Implements the consumer directed services through conducting person centered client assessment, creating individual service plan, conducting follow-up and on-going monitoring of veteran; Provides information and referral services, counseling and other supportive service; Maintains confidential case files.
Serve as a liaison between AACOG, Veterans Administration, Fiscal Management Agencies and social service agencies; Attends meetings, conferences, workshops and other in-service training programs related to the case management program and benefits counseling; Assists in developing the area plan and any necessary amendment program/benefits counseling program; Liaison between AACOG, vendor regarding invoices; Completes related monthly reports; Performs other job related duties as assigned.
Desired Degree/Job Experience: High School Graduate or GED with some college courses in Behavioral Sciences, Social Services, Public Administration or related field; Two (2) years of full-time Aging, Social Services experience can be substituted for one (1) year of preferred education or any equivalent combination of experience and training which provides the required knowledge, skill, and abilities. Military affiliation (past service member, reservist, spouse, family member) a plus but not required.
Other Qualifications: Valid Texas driver's license and reliable transportation required; Travel within 21 counties covered by VA South Texas Health Care System; Provides outreach to those clients who may benefit from other programs developed within the Area Agency on Aging; Implements the case management program through the identification of clients; Proficiency in basic computer skills, Windows, Microsoft programs; Database utilized for maintenance of electronic files; Bilingual Spanish/English highly preferred.
This position is Pay Grade 10, with a starting hourly rate of $20.10. This is a non-exempt position. Open until Filled. Only electronic online applications will be accepted.
