Home Delivered Meals

The Home Delivered Meals program provides nutritionally balanced meals to individuals aged 60 and older who are homebound. Beyond delivering a wholesome meal, this service offers a valuable opportunity for face to face interaction with a caring delivery driver. These visits allow for wellness checks, enabling drivers to monitor participants’ health and overall well-being, ensuring support both nutritionally and emotionally.

Congregate Meals

The Congregate Meals program offers nutritionally balanced meals at designated community locations. In addition to providing healthy meals, these gatherings create a social environment where older adults can connect, share experiences, and build friendships. These meals are often paired with health and wellness programs that empower seniors to manage chronic conditions and take an active role in maintaining their health.

Health Passport Project

A health passport is a tool designed to improve communication and ensure continuity of care between individuals, caregivers, and healthcare providers. It offers a clear summary of a person’s medical history, current conditions, medications, allergies, communication needs, behavioral supports, and personal preferences. For people with IDD who may have difficulty explaining concerns or see multiple providers, the passport helps make sure vital information is available wherever they go.
