Older Adult Services

Assessing the Needs of Adults 60 Years of Age and Older

Care coordination is a personalized service that helps older individuals assess their needs and connect with local resources to support independent living. Priority is given to seniors who have experienced a major illness, health crisis, or hospitalization and need additional support during their recovery process.

Care Coordination services include:

  • Income Support: Assistance with managing utility bills and ensuring financial stability.
  • Health Maintenance: Support for health related needs, including eyeglasses, dental care, incontinence supplies, and more.
  • Residential Repairs: Minor home repairs or modifications to ensure a safe and comfortable living environment.
  • Personal Assistance: Help with daily activities, such as bathing, dressing, and other essential tasks.
  • Emergency Response Systems: Electronic monitoring systems are installed to provide peace of mind and quick response in emergencies.

The Care Coordination team works closely with you and your loved ones to ensure you receive the necessary support during your recovery and beyond, helping you maintain your independence and quality of life.

Who Does AACOG’s Area Agencies on Aging Serve?

AACOG’s two Area Agencies on Aging play a crucial role in supporting older adults and their caregivers throughout the region. The Bexar Area Agency on Aging (Bexar AAA) serves residents of Bexar County, while the Alamo Area Agency on Aging (Alamo AAA) covers a broader region, assisting individuals in Atascosa, Bandera, Comal, Frio, Gillespie, Guadalupe, Karnes, Kendall, Kerr, McMullen, Medina, and Wilson counties.

The services provided by both agencies are available to seniors aged 60 and older, as well as family caregivers who support those 60 and older. Additionally, older relative caregivers, who are 55 or older and raising their grandchildren or other kin, are also eligible for assistance.

Through AACOG’s Aging and Disability Resource Center (ADRC), known as Alamo Service Connection, further support is extended to adults with disabilities of any age, as well as their family members and friends who assist them. The ADRC ensures that people in these categories receive the help they need, regardless of income.

How does the Area Agency on Aging (AAA) help?

AAA services include:

  • Information and referral
  • Community education on aging issues
  • Counseling on Medicare, Medicaid, and Social Security
  • Legal assistance
  • Health and Wellness classes
  • Transportation coordination
  • Medication management
  • Assistance with in home services and minor residential repairs
  • Family caregiver support groups
  • Respite for family caregivers
  • Grandparents raising grandchildren support groups
  • Hot lunches, and home delivered meals for seniors in coordination with community partners
